Woman thrown to her death after staff ‘forget to attach bungee cord’

A 21-year-old woman died after plunging around 130 feet from a bridge during a bungee jump in Brazil.
According to reports, staff allegedly forgot to attach her safety cord before the jump. The fatal incident was captured on video, and authorities have launched an investigation.
